Acute Mitral Regurgitation in Man
نویسنده
چکیده
Four patients are presented who developed acute mitral regurgitation 5 days, 2, 3, and 18 months, respectively, before cardiac catheterization. The hemodynamic findings in the three early cases resemble those found in the presence of a constricting pericardium: left and right ventricular, as well as left and right atrial pressures, equilibrated during diastole. As expected, giant left atrial v waves accompanied markedly elevated pulmonary arterial pressures and low cardiac indices. Biplane angiocardiography showed only slight left atrial and moderate left ventricular enlargement, but showed a large regurgitant fraction and dilated pulmonarv veins. Orthopnea was transient or absent in two of the three early cases. The pericardium may play a role in the early phase of acute mitral regurgitation in mimicking signs of right heart failure and preventing pulmonary edema. Development of increased pulmonary venous capacitance may be another major early mechanism of compensation since the pericardium may be preventing left atrial enlargement. Later in the course, after 6 months, the pericardium does not appear to be a factor in the hemodynamics, as noted in our patient and in patients reported by others.

Introdution: Ischemic mitral regurgitation (IMR) remains one of the most complex and unresolved aspects of ischemic heart disease that the impact of percutaneous coronary intervention (PCI) on improvement of intensity of ischemic mitral regurgitation is not well clarified. Patients with coronary artery diseases and ischemic mitral regurgitation have a worse prognosis than the patients with coro...

A rare complication of AMI is papillary muscle rupture (PMR), occurring in 1% of patients with acute myocardial infarction (AMI). Mitral regurgitation related to PMR occurs at a median of 1 day (ranging from 1-14 days) after onset of myocardial infarction. PMR tends to occur in those having a first time AMI and those without extensive collateral circulation. PMR involves posteromedial papillary...
متن کاملDiastolic Mitral Regurgitation in Intact Mitral Valve Detected by Color Doppler Echocardiography in a Patient with Acute Aortic Regurgitation.
A 38-year-old man was admitted with coingestive heart failure due to infective endocarditis. Echocardiography with color Doppler imaging revealed severe aortic regurgitation, mitral valve premature closure and diastolic mitral regurgitation. The flow of the diastolic mitral regurgitation was directed to the posterior wall of the left atrium through just behind the posterior mitral leaflet. The ...
متن کاملAnterior mitral valve aneurysm perforation in a patient with preexisting aortic regurgitation.
We report the case of a 71-year-old man hospitalized for acute heart failure. Transthoracic and transesophageal echocardiography showed mitral valve aneurysm (MVA) rupture and severe mitral regurgitation. No vegetations but significant aortic regurgitation were also observed. MVA perforation is a rare life-threatening condition that typically occurs as a complication of endocarditis but may als...

BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...

Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
